The following Financial Aid Policy applies to all students at Choffin Career and Technical Center receiving the Pell Grant and/or Direct Student Loans or on payment plans.

MISSION STATEMENT

It is the mission of Choffin Career and Technical Center to develop individuals with technical, scholastic, employability, and life-long learners in cooperation with business, industry, and other community resources.

STATEMENT OF PRINCIPLES

The primary purpose of the financial aid programs shall be to provide financial assistance to accepted students who, without such aid, would be unable to attend Choffin Career and Technical Center.

Choffin Career and Technical Center recognizes its obligation to assist in realizing the national goal of quality educational opportunity. Choffin Career and Technical Center, therefore, works with schools, community groups, and other educational institutions in support of this goal.

Parents of dependent adult students are expected to contribute according to their means, taking into account their income, assets, number of dependents, and other relevant information. Independent students, themselves, are expected to contribute from their own assets and earnings including appropriate borrowing against future earnings.

Financial aid will be offered only after determining that the resources of the family are insufficient to meet the student’s educational expenses. The amount of aid offered will not exceed the amount needed to offset the difference between the student’s educational expenses and the family’s resources according to the following formula:

Cost of Attendance (COA) - Expected Family Contribution (EFC) = Financial Need

Because the amount of financial assistance awarded reflects the economic circumstances of the student and his/her family, the school will refrain from any public announcements of the amount of aid awarded and encourage the student and others to respect the confidentiality of this information.

All documents, correspondence, and conversations between and among the aid applicant, his/her family, and financial aid officers are confidential and entitled to the protection ordinarily arising from a counseling relationship.

CONFIDENTIALITY

Student financial aid records are considered confidential and hence are only available to authorize financial aid personnel for the purpose of making and maintaining financial aid awards.

Students who transfer from this institution into another post-secondary institution may be required to have a Financial Aid Transcript sent to the new school. Students who apply for food stamps or other aid awards must request in writing a release of financial aid information.

STUDENT RECORDS

Student financial aid records are kept on file at Choffin Career and Technical Center following graduation. These records include, but are not limited to: academic progress, attendance, grade reports, Work Key scores, resumes, financial aid application, ISIR, supporting financial aid information and billing records..

Students may request in writing copies of their records or may make an appointment to review records with the Financial Aid Officer in the Adult Education office. The record must be reviewed in the presence of the Financial Aid Officer or designee. The student must sign a release prior to the file review.

A student may have a representative review the record with him/her.

TYPES OF FINANCIAL ASSISTANCE

Choffin Career and Technical Center participates in the following U.S. Department of Education student financial aid (SFA) programs:

  • Federal Pell Grant (Title IV)
  • Direct Student Loans

Pell Grant

Is financial aid that does not have to be re-paid. The Pell Grant is awarded based on the student’s financial need. Any student wishing to receive a Federal Pell Grant at this institution must meet the eligibility requirements established by the U.S. Department of Education. To receive a Federal Pell Grant a student must complete the Free Application for Federal Student Aid (FAFSA) at https://fafsa.ed.gov . Students with a Bachelor’s Degree are not eligible for a Pell Grant.

Direct Student Loan

Subsidized Direct Loan

This loan is awarded on the basis of need. The U.S. Department of Education pays the interest while the borrower is in school, during the grace period and authorized periods of deferment. Unsubsidized Direct Loan

For undergraduate and graduate students; borrower is responsible for all interest; student must be at least half time; financial need is not required.

Direct Plus Loan

For parents of dependant undergraduate students and for graduate or professional students; who attend at least half time; financial need is not required. For further questions visit https://studentaid.gov

Business or Agency Sponsorship

Choffin Career and Technical Center works with various businesses and agencies that fund qualified applicants for training programs. The school also works with businesses that have training dollars for their employees and various tuition assistance plans. Students using such funding for tuition payment must meet with the Financial Aid Officer to complete the registration process.

Department of Veteran Affairs (VA) Veterans Administration

Federal training money is available to veterans of the U.S. Armed Services. For eligibility visit https://benefits.va.gov/gibill Student must complete the appropriate application, submit their Certificate of Eligibility letter to the Financial Aid Office and then the Financial Aid Office will certify enrollment once the student has started classes.

Trade Adjustment Assistance (TAA)

Assistance may be available for students who have lost jobs or had hours and wages reduced because of foreign trade. Contact your employer, local Ohio Bureau of Employment Services office or local One Stop location.

Workforce Investment Act (WOIA)

Assistance may be available for students who are economically disadvantaged or dislocated workers. Contact your local One Stop for information and eligibility requirements or school financial aid office.

Bureau of Vocational Rehabilitation (BVR)

Financial aid may be available from this local agency for students with physical, mental or emotional disabilities that present a handicap to employment. Contact your local area Bureau of Vocational Rehabilitation.

Self-Pay Payment Plan

A self-pay, interest-free payment plan is available to assist those students paying all or a portion of their tuition expense. Application for the payment plan is available through the Financial Aid Office. Payments must be made on time according to the individualized payment plan to avoid a $25 per month administrative fee. Student tuition account must be paid in full at the end of each payment period, prior to any externship, and in order to graduate.

STUDENT ELIGIBILITY

To receive aid from major student aid programs, a student must:

1. Have financial needs;

2. Have a high school diploma or a General Education Development (G.E.D.) Certificate;

3. Be enrolled or accepted for enrollment as a full-time student working toward a certificate;

4. Be a U.S. Citizen or eligible non-citizen;

5. Have a valid Social Security Number;

6. Make satisfactory academic progress;

7. Sign a statement of educational purpose and a certification statement on overpayment and default (both found on Free Application for Federal Student Aid (FAFSA); and

8. Register with the Selective Service (males only).

WHAT WILL MY EDUCATION COST?

Aid from most major programs is awarded on the basis of financial need with the exception of unsubsidized Direct Student Loans. When applying for Federal student aid, the information reported is used in a formula established by the U.S. Congress that calculates Expected Family Contribution (EFC), (an amount a student and his/her family are expected to contribute toward education). If the EFC is below a certain amount, a student will be eligible for a Federal Pell Grant, assuming he/she meets all other eligibility requirements. The EFC is used in an equation to determine financial need:

The school’s Financial Aid Officer (FAO) calculates cost of attendance (COA). Cost of attendance includes: tuition and fees; books/supplies; transportation; room and board; personal expenses; dependent daycare. The amount a student and his/her family are expected to contribute toward that cost is subtracted from COA. If there is an amount left over, a student is considered to have financial need. In determining need for aid from the Student Financial Aid Programs, the FAO must first consider other aid that a student might receive.

The Financial Aid Administrator puts together a financial aid package that comes as close as possible to meeting a student’s need utilizing the types of financial assistance the student is eligible for. However, the amount awarded may fall short of the total program costs. The balance of costs is the student’s responsibility.

FAFSA APPLICATION PROCESS

It is the students responsibility to file a FAFSA and receive the SAR (Student Aid Report). The school cannot determine an award without this information. You do not need to see Financial Aid to begin. Choffin Career and Technical Center School Code is: 007473.

Step 3. You will receive an email confirmation once your application has been completed.

Step 4. Set up an appointment with Financial Aid at 330-744-8703.

DEPENDENCY STATUS

When applying for Federal student aid, answers to certain questions will determine whether a student is considered dependent upon his/her parents. If dependent, a student must report their parents’ income and assets as well as his/her own. If independent, a student must report only his/her own income and assets (and those of a spouse if married).

Students are classified as dependent or independent because Federal Student Aid (FSA) programs are based on the idea that students (and their parents or spouse, if applicable) have the primary responsibility for paying for their post-secondary education. Students who have access to parental support (dependent students) should not receive need-based federal funds at the expense of students who do not have such access (independent students).

A student is considered dependent if all of the following apply:

1. He/she is under age 24,

2. Not married, and

3. Has no dependent children.

Unusual circumstances will be considered on an individual basis.

A student is considered independent if at least one of the following statements applies to the student:

1. Has reached the age of 24 prior to January 1st of the upcoming year;

2. Married as of the date the FAFSA was submitted;

3. Enrolled in a graduate or professional educational program (beyond a bachelor’s degree);

4. Have legal dependents other than a spouse that receive more than half their support from you;

5. An orphan or ward of the court (or were a ward of the court until age 18);

6. Currently serving on active duty in the U.S. Armed Forces (other than training); or

7. A veteran of the U.S. Armed Forces. (“Veteran” includes a student who attended a U.S. military academy who was released under a condition other than dishonorable)

If a student under age 24 believes he/she may qualify as an independent student, the school will ask for written proof before applying for or changing the status of an application for federal student aid. If there are unusual circumstances that might classify a student as independent, even though none of the above criteria apply, the student must prove independence by providing any additional documentation requested by the Financial Aid Officer. The required documentation must be submitted to the Financial Aid Officer within 15 working days of the request. The Financial Aid Officer will notify the student of his/her status within 10 working days.

VERIFICATION

The CPS (Central Processing System) selects which applications are to be verified. Choffin Career and Technical Center is also required to verify the accuracy of any FAFSA applications, whether selected by the CPS or not, believed to be inaccurate, containing conflicting and/or incomplete information. It is the policy of Choffin Career and Technical Center that all applications selected for verification, either by the CPS or Choffin Career and Technical Center, must be verified, and all conflicting information must be resolved before disbursing FSA funds.

All documents required for verification must remain on file at the Choffin Career and Technical Center Financial Aid Office. Fifteen (15) working days are given to provide verification documentation.

If verification is required and documents are not provided in the allotted timeframe, the student will not receive aid from the FSA programs and might not receive aid from other sources.

The documentation needed for verification varies according to the item verified. The Financial Aid Administrator will advise students and parents on the documentation to be submitted. Students and/or parents of dependent students must also sign a Verification Worksheet certifying the accuracy of the information being reported.

The U.S. Department of Education encourages students and parents who complete the FAFSA on the Web to use the IRS Data Retrieval process to import data from their tax return and not change it. For retrieved data to be acceptable documentation of tax data, it is necessary that neither students nor parents change the data after it is transferred from the IRS. The IRS Data Retrieval Tool is the fastest, easiest, and most secure method of meeting verification requirements.

Through the verification process, if any FAFSA information is found to be in need of updating or correcting, the student will be notified either in person or via a phone conversation. Corrections and updates can be submitted by the student on the SAR or the Web or by the school using FAA Access to CPS Online or the Electronic Data Exchange.

If corrections made result in a change in the student’s EFC and Title IV aid amounts, the student would be notified via phone, and an appointment will be set with the Financial Aid Officer to review and sign an updated Award Letter.

If discrepancies are found after disbursing FSA funds, the conflicting information must still be reconciled and appropriate action taken to correct the FAFSA information. If corrections result in a change in the student’s EFC and Title IV aid amounts, the student will be notified as stated above. The student will also be notified in writing if they are required to return any Title IV funds they may have already received.

Fraud

Students, parents, or other individuals who purposely give false or misleading information to fraudulently obtain federal funds may be fined, be sentenced to jail, or both. If misreported information or altered documentation is suspected, the submitting parties will be asked to resubmit accurate, unaltered documentation for verification. If the parties do not wish to resubmit, they must present in writing a signed letter stating the information and/or documentation they are providing is accurate, complete, and without alterations. Individuals still suspected of misreporting information or altering documentation will be reported to the Office of Inspector General U.S. Department of Education, and any evidence will be provided.

DIRECT LOAN APPLICATION PROCESS

Direct loans may pay only for education costs not covered by other sources of financial aid. Because this is not free money, students should research cost-free options such as grants, scholarships, and other sources of aid before requesting and accepting an education loan.

Students may qualify for a Direct Loan if:

1. You are a U.S. citizen, national, or eligible non-citizen, and provide your valid social security number;

2. You are enrolled at least half-time at an eligible institution and maintain satisfactory academic progress (as determined by your school);

3. You are not in default on any federal education loan unless satisfactory arrangements to repay have been made;

4. You do not owe a refund on any Title IV grant.

5. You do not have student loans in active bankruptcy.

Students applying for a Federal Direct loan must complete the following steps:

1. File a FAFSA online at. https://fafsa.ed.gov

2. Complete a Master Promissory Note (MPN) online at https://studentloans.gov The MPN is a contract that legally obligates you to repay your student loan. It is important that you complete and thoroughly understand the paperwork, and that you agree with the terms of the loan as described in the MPN packet. Keep a copy of all paperwork for your records.

3. Complete required Entrance Counseling session online at https://studentloans.gov

4. Student or the Federal Department of Education submits printed proof of completed MPN and Entrance Counseling to the Financial Aid Office.

5. Notice of loan guarantee sent, loan disbursed. After the loan is guaranteed, A Notice of Guaranty and Disclosure will include all loan information such as amount of the loan, interest rate, origination fee, and an anticipated disbursement schedule. If money remains after the bills are paid, the school will credit the difference to the student.

6. Complete required online Exit Counseling session for loan repayment regarding their responsibility before the end of the current academic year. 7

If after making every effort to resolve a dispute about your federal student loan with either your student loan servicer or school, the FSA Student Loan Ombudsman’s Office is available to student loan borrowers for further inquiry. The Ombudsman Group is a neutral, informal, and confidential resource to help resolve disputes about your federal student loans.

AID DISBURSEMENT

If a students’ FAFSA application is selected for verification, aid will not be disbursed until verification is complete and any conflicting information is corrected. Initial loan funds will not be disbursed until the student has completed the on-line Entrance Counseling and Master Promissory Note. Aid is applied to the student’s account as it is received to cover books, tuition and supplies. When all aid is credited the same day, Pell Grant funds are considered to be credited first. Total aid received from all sources cannot exceed program Cost of Attendance. Any resulting credit balance for the payment period is refunded to the student after processing through the Treasurer’s Office. Allow up to 14 days from the date the credit balance occurred on the student account to receive a refund check.

Title IV aid for Career Certification Programs of 900 or more hours in length are entitled to the maximums set forth by the Federal Government.

Programs are divided into two terms/payment periods within an academic year. Aid will be disbursed in two (2) equal installments – one (1) installment each payment period. If a program crosses over into a new academic year, an additional payment period may be necessary to cover the remaining Certification Program hours and students will need to renew their FAFSA for the additional payment period.

In order for a student to be eligible for the next financial aid disbursement and to maintain the next payment period’s disbursement date, the Financial aid Officer will determine that the student has successfully attended both the clock hours and the weeks of instruction time required for the previous payment period(s). Students who have not yet attended the required clock hours and weeks will delay their anticipated disbursement date.

CHANGE OF CIRCUMSTANCES

Students are responsible to report any significant changes of circumstances to the Financial Aid Officer including enrollment status, permanent address, phone number and email.

SATISFACTORY ACADEMIC PROGRESS AND ATTENDANCE (SAP)

Satisfactory academic progress is a requirement for all students enrolled. Federal regulations require all schools participating in state and federal financial aid programs to monitor SAP.

Academic Year: Choffin Career and Technical Center is a (10) month clock hour program. This is divided into two semesters that are 20 weeks long. For Title IV payments the student must meet both clock hours and weeks of instruction as well as complying with all standards for Satisfactory Academic Progress before they can receive further Title IV payments. All periods of enrollment count toward SAP (Fall and Spring), including periods when a student does not receive Title IV aid. Choffin does not offer summer semesters. All programs (Practical Nursing, Surgical Technology and Dental Assisting) are run in a continuous 20 week –terms with a Christmas Break and a Spring Break.

A student’s academic progress evaluated each grading period (Fall and Spring) in which the student is enrolled. All students enrolled in adult program(s) at Choffin Career and Technical Center must attain satisfactory academic grade and attendance, to ensure progress towards certification/graduation in their designated program. SAP will be measured before each payment period (semesters 1&2) before any subsequent disbursements can be made. To receive subsequent disbursements of aid, the student must be meeting the SAP policy for both attendance and grades.

All students receiving Federal Financial Aid including Pell Grants and Federal Student Loans must maintain satisfactory progress according to the following standards:

Pace of Instruction

Since program start dates occur only once per academic year, students are expected to start and complete a program within the scheduled time. Therefore the maximum time frame and pace of instruction is 90% of the scheduled hours. If a student withdraws from a program and wishes to return, he/she will need to wait until the next academic year to restart the program.

Quality (grades)

Students in Surgical Technology and Dental Assisting must maintain a cumulative grade average of 75%. The Practical Nursing students must maintain a cumulative grade average of 80%.

Time Frame

Students must complete the program within 111% of the normal length of the program.

Completion Rate

Student must maintain a 90% attendance percentage average to complete his/her program. Students are allotted ten absences in each of the (10) month programs (Practical Nursing, Surgical Technology and Dental Assisting), once a student misses 10 or more days the student is terminated. No financial aid is given.

Remedial Courses

Choffin does offer remedial classes though ASPIRE, but does not grant credit for remedial courses. Remedial courses will count toward the maximum time frame. 9

Repeat Courses

When a course is repeated the new grade replaces the old grade. The hours in both courses are counted in the time frame and for attendance percentage. Students are only allowed to repeat course one time only.

Incomplete

A grade of incomplete is not counted in the grade point average. If the student does not complete the work in the specified time the incomplete will change to an F, which negatively impacts the grade point average but it is also the maximum time frame. If the grade changes it counts for the GPA and maximum time frame.

Withdrawal or Termination

A student may withdraw for any reason and complete a withdrawal form. The student must meet with the financial aid coordinator to review any outstanding tuition and fees. Re-admission to the individual programs must be reviewed with the individual program director.

Note: The term “withdrawal” defines any reason for a student leaving school, whether it is initiated by the student or by the school.

Students receiving the Pell Grant, Direct Student Loans and/or other financial assistance who are planning to withdraw should meet with the Financial Aid Administrator (FAA) before exiting from a Career Certification Program. Students who are unable to physically meet with the FAA must contact the FAA and submit a written letter of withdrawal. Regardless of circumstances, students who are withdrawn or terminated from a Pell-eligible Career Certification Program are subject to the U.S. Department of Education’s Return of Title IV Funds Policy.

When a student is withdrawn, the student may no longer be eligible for the full amount of Title IV funds that the student was originally scheduled to receive. This means the student may end up owing money to Choffin Career and Technical Center and/or the Department of Education. It is the student’s financial responsibility to pay any remaining balance to the school for the total program tuition based on the school’s refund policy.

When calculating Title IV funds, the official withdrawal date will be the last day of classroom attendance as indicated on the classroom sign in sheet. Federal returns will be made within 45 days of the determination date. If a student has a federal loan, it is his/her responsibility to inform the loan servicer that he/she is no longer in school. It is also the student’s responsibility to complete federal loan exit counseling online at https://studentloans.gov

Attendance

The student’s training program is closely monitored. Student’s are allotted 10 absent days for the length of his/her program.(All programs are 10 months). Each student is responsible for tracking his/her own absences and tardiness. All students are expected to establish an excellent attendance record that will be an asset in job search. Each program maintains a record of absences and early departures, and tardiness, anything over the 10 days absent before the 10 month program is complete will result in termination.

Attendance Advisement

Attendance will be reviewed and students are advised if they are at risk for termination due to lack of continuous progress for program completion. Students will receive an advisement form when their attendance meets or exceeds 5 days. If a student misses 10 days of lab and theory combined, before academic year is over, the result is termination from the program.( the school will notify the participating agencies to terminate payment of educational benefits).

Leave of Absence

Choffin Career & Technical Center does not grant official leave of absence. Therefore, a student who is unable to attend classes for an extended period of time must withdraw from his/her program and reapply when circumstances allow for re-entry.

Appeal

A student may submit an appeal in the form of a letter along with any supporting documentation to the Adult Education Supervisor describing circumstances beyond the student’s control that may have caused a failure to meet the satisfactory progress standards. The Adult Supervisor will review the appeal and a determination of the appeal will be made within two weeks. Cases will be reviewed on an individual basis and the student will be notified of his/her appeal status. If the appeal is approved, financial aid will be reinstated.

Academics

A satisfactory academic average is critical for students to be eligible to take licensing/certification exams and to participate in externships. Classroom performance is based upon classroom participation, lab, written work, tests and evaluations. Students who fall below the expected 75%(Dental and Surgical) or 80% (Practical Nursing) academic average during a semester meet with their instructor for advisement. All faculty is available to advise students about grades, clinical performance and provide tutoring. One, fifteen minute, time slot is available each class day to talk with faculty or the Program Directors of Practical Nursing, Dental Assisting or Surgical Technology. A sign-up sheet is posted in department. Other times for talking with faculty can be arranged on an individual basis. If a student’s academic average does not improve by the end of the semester, it is recommended, through advisement, that the student with draw and re-enter the next program year. If the student refuses to withdraw, the student is unable to continue and will be terminated from the program.

Re-entry

Choffin Career and Technical Center programs are comprehensive, students are expected to start and complete his/her program within the 10 month, scheduled time frame. If a student withdraws and wishes to return to his/her program, he/she will need to wait for the new start date to re-apply.

Withdrawn students who have submitted a re-entry request to Choffin Career and Technical Center prior to orientation of the next class and have a zero tuition account balance are permitted to re-enter the same program one time.(All requirements must be up to date).

Re-entry to the Practical Nursing program at the start of the second semester is applicable for students who successfully completed the first semester (>80% in all courses). Also that they have successfully completed all other requirements (current BCI/FBI checks, up to date immunizations, current CPR/First Aid certificates). All re-entry applications are subject to approval at the discretion of the Adult Education Director.

A student who becomes ineligible for aid will be responsible for paying all fees that were not covered by financial aid.

Change in major/Second major

Choffin Career and Technical Center’s programs (Practical Nursing, Surgical Technology and Dental Assisting) are 10 month clock hour programs and do not provide change in major or second major. (example) A student who is in Surgical Technology and wants to go into Practical Nursing would either have to complete Surgical Tech or withdraw from Surgical Tech and wait for Practical Nursing to start the following year.

Transfer Credits

Choffin Career and Technical Center does not accept credit transfers.

REFUND POLICY FOR DEPOSIT

100% of the deposit will be refunded if student withdraws within 3 days of placing initial deposit. 50% of the deposit will be refunded if student withdraws after 3 days of placing initial deposit and before the start of the first day of class. This refund will be received within forty-five days of initial student request.

All application and testing fees are non-refundable.

Refund of tuition is calculated for withdrawing students based on the last date of classroom attendance. Student can withdraw from CCTC (officially i.e. with notification or unofficially without notification). Refund of tuition is based on federal policy for Return to Title IV fund (R2T4). Refunds will be received within 45 days of last date of classroom attendance. A student withdrawing after attending 3 weeks or more will be obligated for 100% of the tuition that semester.

Tuition and refunds will be calculated based on two equal time periods depending on the length of the program.

RETURN of TITLE IV POLICY

A student recipient of Federal Title IV funds (i.e., Unsubsidized or Subsidized Federal Direct Loans, Federal Pell, or Plus loans) who withdraws from Choffin before completing 60% of the term is subject to the R2T4 calculation. Choffin Career and Technical Center’s programs are clock hour programs. Choffin is required to determine the earned and unearned portions of Title IV aid based on the last date the last date of academic attendance.

Students receiving the Pell Grant, Direct Student Loans and/or other financial assistance who are planning to withdraw should meet with the Financial Aid Administrator (FAA) before exiting from a Career Certification Program. Students who are unable to physically meet with the FAA must contact the FAA and submit a written letter of withdrawal. Regardless of circumstances, students who are withdrawn or terminated from a Pell-eligible Career Certification Program are subject to the U.S. Department of Education’s Return of Title IV Funds Policy.

When a student is withdrawn, the student may no longer be eligible for the full amount of Title IV funds that the student was originally scheduled to receive. This means the student may end up owing money to Choffin Career and Technical Center and/or the Department of Education. It is the student’s financial responsibility to pay any remaining balance to the school for the total program tuition based on the school’s refund policy.

When calculating Title IV funds, the official withdrawal date will be the last day of classroom attendance as indicated on the classroom sign in sheet. Federal returns will be made within 45 days of the determination date. If a student has a federal loan, it is his/her responsibility to inform the loan servicer that he/she is no longer in school. It is also the student’s responsibility to complete federal loan exit counseling online at https://studentloans.gov

Official Withdrawal

An “Official Withdrawal” is when a student completes a withdrawal form. The last date of attendance will be used for the calculation of Return of Title IV funds.

Unofficial Withdrawal

An “Unofficial Withdrawal” is when a student has been absent for more than 10 days and the last date of academic attendance will be used for the Return of Title IV funds.

Up through the 60% point in each payment period or period of enrollment, a pro rata schedule is used to determine the amount of Title IV funds the student has earned at the time of withdrawal. After the 60% point in the payment period or period of enrollment, a student has earned 100% of the Title IV funds he/she was scheduled to receive during the period.

For a student who withdraws after the 60% point in time, there are no unearned funds. However, a school must still determine whether the student is eligible for a post –withdrawal disbursement (PWD).

(Percent earned= number of scheduled hours completed up to the withdrawal date divided by the hours in the payment period).

(Percent unearned= 100% minus percent earned).

When a student receives federal financial aid in excess of aid earned then:

The school returns the lesser of: Institutional charges multiplied by the unearned percentage or Title IV federal financial aid disbursed multiplied by the unearned percentage.

The student returns: Any remaining unearned aid not covered by the school- Any loan funds are repaid in accordance with the terms of the promissory note, (Scheduled payments to the holder of the loan over a period of time). Any grant amount the student has to return is considered a Federal Title IV grant overpayment and arrangements must be made with the school or the U.S. Department of Education to return the funds.

Post withdrawal Disbursement

If it is determined that the student has not received all of the “earned” federal aid for which the student is entitled, then the student will receive a post-withdrawal disbursement of aid funds. Grant funds will be applied to their tuition account to settle any tuition still owed the school as a result of the student’s withdrawal. If funds are due from the federal loan program, the student will be notified within 10 days of the date the school determined that the student withdrew, and the student will need to respond within 10 days. The student or in the case of a Plus Loan, will need to request the funds in writing before the loan disbursement will be made. If the student or parent cannot be located, any credit balance must be returned to the applicable Title IV program.

The school will return Title IV funds to programs from which the student received aid during the payment period or period of enrollment, as applicable, in the following order up to the amount disbursed from each source:

  • Unsubsidized Federal Direct Loans
  • Subsidized Federal Direct Loans
  • Plus
  • Federal Pell Grants for the payment period for which a return of funds is required

All returns owed to the Federal Financial Aid Programs will be processed within 45 days.

CREDIT BALANCE POLICY

All Federal Student Aid funds including disbursements from Pell Grant and Federal Direct Loan programs will be credited to the student’s account for charges including tuition, fee, books and kit. After a student‘s account is paid in full, if a credit balance occurs Before the student graduates, the credit balance will be paid to the student as soon as possible but no later than 14 calendar days.

STUDENT RIGHTS AND RESPONSIBILITIES

As a recipient of Federal Student Aid, a student has certain rights he/she should exercise, and certain responsibilities that must be met. Knowing what they are will enable the student to make better decisions about educational goals and how to best achieve them.

Student Rights:

1. The right to know what financial aid programs are available at the school.

2. The right to know the deadlines for submitting applications for each of the financial aid programs available.

3. The right to know how financial aid will be distributed, how decisions on the distribution are made and the basis for these decisions.

4. The right to know how financial need was determined. This includes how costs for tuition and fees, books and supplies, and miscellaneous expenses are considered.

5. The right to know what resources (such as parental contribution, other financial aid, assets, etc.) considered in the calculation of need.

6. The right to know how financial needs (determined by the institution) will be met.

7. The right to request an explanation of the various programs in the student aid package.

8. The right to know the school’s refund policy.

9. The right to know how the school determines whether a student is making satisfactory progress and what happens if he/she is not.

Student Responsibilities:

1. All application forms must be completed accurately and submitted on time to the Financial Aid Office.

2. Correct information must be provided. In most instances, misreporting information on financial aid application forms is a violation of law and may be considered a criminal offense, which could result in indictment under the U.S. Criminal Code.

3. All documentation, verification, corrections and/or new information requested by either the financial aid office or the agency to which you submitted an application must be provided in writing.

4. A student is responsible for reading and understanding all forms that are to be signed and for keeping copies of them.

5. Responsibility must be accepted for all agreements that are signed.

6. Be aware of and comply with the deadlines for application or reapplication for aid.

7. Be aware of the school’s refund procedures.

8. Carefully consider the information which the school provides about its program and performance to prospective students before deciding to attend.

9. Attend Entrance and Exit meetings pertaining to financial aid.

10. Complete required online MPN, Entrance and Exit Counseling and submit documentation to the Financial Aid office of your school showing successful completion.

11. Promptly respond to requests made by the financial aid office.

Program Termination

The Director and or Administrator have the authority to dismiss any student who violates the school’s rules, policies, and regulations. A record of termination will be maintained.

Class Cancellation Policy

Choffin Career and Technical Center, Adult Education Program reserves the right to cancel, discontinue, postpone, or combine classes. Classes require a minimum enrollment and maybe cancelled due to insufficient enrollment.

Cancellation Policy

Students who have not visited the school prior to enrollment will have the opportunity to withdraw without penalty within 3 business days following a scheduled orientation or after a tour of school. All money paid except for registration fee, will be refunded if requested, within 3 days after signing an enrollment agreement.

Confirmation of Citizenship

All students applying for Federal Student Aid must be able to confirm their citizenship status with the Social security Administration. The conformation process is done when student submits his/her FAFSA. The Federal processor performs a match with the SSA to confirm a student’s citizenship status and social security number. If SSA cannot confirm citizenship status, a comment will appear on the students SAR/ISIR.

Family Educational Rights & Privacy Act (FERPA)

The purpose of the Family Educational Rights & Privacy Act (FERPA) is to protect the privacy of student information contained in student records. Specifically FERPA restricts family members and spouse from inspecting student information without written consent by student. The authorization will remain in student file.

Voter Registration

The U.S. Department of Education requests us to make you aware of your voter rights and how to register to vote. To be eligible to vote in the next election in Ohio, you must be registered at least 30 days before the election date. You can register to vote through the County Board of Elections or at your local Ohio BMV office. You cannot register online.
